When seeing pigmented macrophages of the lung, look for:

Signs of anthracosis (black arrow shows interstitial anthracotic pigment). Nearby macrophages (white arrow) can be presumed to contain anthracotic pigment. Reporting is optional in mild cases in the elderly.
